2) DATE OF BIRTH — we use this to choose the sign (e.g. March 14 1990)
3) PLACE OF BIRTH (for the star map — e.g. 'Dallas, TX' or
'Albany, NY')
4) TIME OF BIRTH (for the star map — e.g. '3:45 PM')
5) COLOR PREFERENCE — list up to 4 filament names from the swatch photo,
or write 'use standard sign colors' to keep the default palette
